Alla fine del Corso lo studente dovrà aver acquisito le conoscenze fondamentali sulle basi cellulari e molecolari della risposta immunitaria innata e di quella adattativa, sulla loro integrazione dinamica e sui meccanismi di controllo delle risposte. Dovrà, altresì, conoscere le principali patologie causate da anomalie delle cellule immunocompetenti e/o delle loro interazioni.

Prove “in itinere”, come prova scritta con domanda per risposta aperta. La prova scritta consiste di 25 domande aperte per valutare la capacità di risposta dello studente e le conoscenze di base della teoria dell'immunologia. Lo studente ha 60 minuti in totale per rispondere. Le domande riguardano gli aspetti di base dei meccanismi cellulari e molecolari dell'immunologia, nonché i principali aspetti dell'immunopatologia. Le domande saranno specifiche per valutare le conoscenze di base dell'argomento. Ciò significa che lo studente deve conoscere i passaggi chiave della risposta immunitaria, i più rilevanti dell'immunità adattativa e innata.

Le competenze degli studenti vengono valutate attraverso diverse prove di valutazione durante il corso e un esame orale finale (viva voce). La prova scritta consiste in circa 32 domande a risposta multipla.
L'esame orale copre tutti gli aspetti teorici dell'immunologia. Gli studenti devono essere in grado di discutere l'argomento e di collegare e analizzare le diverse parti della materia.
Inoltre, gli studenti devono avere una conoscenza approfondita delle risposte immunitarie più comuni, ovvero essere in grado di collegare componenti e meccanismi immunitari alle malattie. Per testare questa capacità, agli studenti verrà chiesto di risolvere semplici problemi immunologici o immunopatologici, come il riconoscimento dei meccanismi immunologici alla base delle risposte immunitarie innate o adattative, ecc. Il voto finale è espresso su una scala di 30/30, dove 18 è il punteggio più basso e 30 il punteggio più alto.

IMMUNOLOGIA
Aspetti generali della risposta immunitaria: risposta innata e acquisita (umorale e cellulo-mediata). Basi cellulari e molecolari della risposta immunitaria. Origine della specificità e della diversità dei linfociti. Selezione clonale.
Organizzazione del sistema immunitario: caratteristiche generali degli organi, dei tessuti e delle cellule. Ontogenesi, differenziazione, caratteristiche fenotipiche e funzionali delle popolazioni e sottopopolazioni linfocitarie. Interazioni cellulari necessarie per generare una risposta immunitaria. Sistemi di comunicazione tra le cellule (molecole di adesione, recettori di homing, citochine) Organi linfoidi primari e secondari. Distribuzione e ri-circolazione delle cellule immunitarie. Organogenesi dei linfonodi.
Immunità innata: barriere anatomiche, fisiologiche e biochimiche, fagocitiche e infiammatorie. I recettori dell'immunità innata. I profili molecolari associati ai patogeni (PAMP) ed i recettori dei profili molecolari (TLR, NLR, RLR ecc.). Fagocitosi: aspetti cellulari e molecolari della fagocitosi i e killing intracellulare (meccanismi ossigeno e azoto-dipendenti e indipendenti) dei neutrofili e dei macrofagi. I linfociti dell’immunità innata (ILC1,2,3). Le cellule Natural killer (NK): origine e caratteristiche fenotipiche. Riconoscimento delle cellule bersaglio. Recettori inibitori e stimolatori. Meccanismi litici. Interazioni tra immunità innata e acquisita.
Il complemento nella risposta innata e specifica: genetica e struttura molecolare dei componenti. Meccanismi di attivazione. La via classica, la via alternativa e via lectinica. Il controllo dell’attivazione. Funzioni biologiche litiche e non litiche.
Riconoscimento dell’immunità specifica
L’antigene: concetti di antigene, immunogeno, determinante antigenico o epitopo, carrier, aptene. Caratteristiche fisico-chimiche dell’antigene come molecola immunogenica. Antigeni timo-dipendenti e timo-indipendenti, superantigeni.
Il recettore per l’antigene dei linfociti B: le immunoglobuline. Struttura e funzioni della molecola solubile (anticorpo) e del recettore di membrana per l’antigene dei linfociti B (BCR). Il sito combinatorio. Isotipia, allotipia, idiotipia. Biosintesi e catabolismo. Le classi e sottoclassi differenze strutturali e funzionali. Gli anticorpi monoclonali: concetto, metodologia, applicazioni. Evoluzione della superfamiglia delle immunoglobuline.
La reazione antigene-anticorpo: affinità, avidità, specificità, cinetica della reazione antigene-anticorpo. Metodiche per lo studio dell’interazione antigene-anticorpo: reazioni indirette (agglutinazione, precipitazione, deviazione del complemento) e dirette (immunofluorescenza, RIA, ELISA, immunoblot). Citometria a flusso attivata dalla fluorescenza (FACS).
Il recettore per l’antigene dei linfociti T: Struttura molecolare dei prodotti genici del TCR e CD3
Le molecole del Complesso Maggiore di Istocompatibilità (MHC): organizzazione genica e polimorfismo. Struttura molecolare e classificazione dei prodotti genici (MHC di classe I e II). Struttura e funzione del solco combinatorio. Ruolo delle molecole MHC di classe I e II nella presentazione dell’antigene. Molecole presentanti l’antigene non convenzionali.
La presentazione dell’antigene al sistema immunitario. Riconoscimento dell’antigene nativo da parte dei linfociti B e riconoscimento MHC-ristretto da parte dei linfociti T. Sviluppo del repertorio linfocitario B e T: meccanismi cellulari e molecolari che operano dal precursore staminale al linfocita T o B maturo vergine. Principali fattori del differenziamento cellulare antigene indipendente. Selezione positiva e selezione negativa dei linfociti T nel timo. Selezione dei linfociti B nel midollo osseo. I marcatori ed i metodi di studio dello sviluppo linfocitario.
Meccanismo d’azione dei recettori dell’immunità
Attivazione dei linfociti T e B: Definizioni e concetti generali. I sistemi di trasduzione del segnale dei recettori per l'antigene. Meccanismi molecolari coinvolti ed i fattori trascrizionali nella proliferazione e differenziazione antigene-dipendente. Principali molecole accessorie, di molecole di adesione ed i principali sistemi di costimolatori e coregolatori che partecipano al processo di attivazione linfocitaria.
Le citochine ed i loro recettori: origine e struttura molecolare. Le caratteristiche generali delle citochine. L'espressione delle citochine ed il meccanismo d'azione delle citochine nell'attivazione trascrizionale dei geni bersaglio: le vie di trasduzione ed i fattori trascrizionali principali coinvolti. Il cross-talk, o integrazione, delle vie di trasduzione dell’immunità innate e specifica.
Meccanismi effettori dell’immunità specifica
Meccanismi effettori dell’immunità umorale. La cooperazione tra linfociti T helper e linfociti B: i meccanismi cellulari e molecolari dell'attivazione linfocitari B T dipendente e indipendente. Cinetica della risposta primaria e di quella secondaria. Fasi follicolari e stadi della maturazione cellulare. Le sottopopolazioni dei linfociti B. Lo switch isotipico, maturazione dell’affinità degli anticorpi. Le funzioni cellulari anticorpo-mediate. Distribuzione cellulare dei recettori per Fc. La riposta IgE mediata. Meccanismi effettori dell’immunità cellulo-mediata. L’attività macrofagica controllata dai linfociti: attivazione e funzione macrofagica classica di tipo 1 e macrofagica alternativa di tipo 2. I linfociti T citotossici (CTL): attivazione da linfociti pre-citotossici a linfociti citotossici effettori. Fasi e meccanismi molecolari del killing della cellula bersaglio da parte dei CTL. IMMUNOPATOLOGIA
Le reazioni di ipersensibilità o immunopatogene: classificazioni. Meccanismi e principali manifestazioni delle reazioni anafilattiche, citolitiche o citotossiche e da immunocomplessi. Ipersensibilità ritardata.
L’autoimmunità e le malattie autoimmuni: ipotesi eziopatogenetiche più accreditate. Classificazione generale delle malattie autoimmuni. Rapporti tra fenotipo HLA e frequenza di malattie autoimmuni. Basi Sperimentali della immunoregolazione.
Immunologia dei trapianti: tipizzazione degli antigeni di istocompatibilità. Trapianti di tessuti, organi e cellule emopoietiche. Meccanismi di rigetto. Graft-versus-host disease (GVHD). Tecniche per lo studio della istocompatibilità dei trapianti d’organo.

At the end of the course the student must have acquired the fundamental knowledge on the cellular and molecular bases of the innate and adaptive immune responses, their dynamic integration and the mechanisms of control of the responses. He will also have to know the main pathologies caused by abnormalities of the immunocompetent cells and/or their interactions.

Tests “in itinere”, as written test with question for open answer. The written test consists of 25 open asks meant to evaluate the student’s ability to answer and the basic knowledge of Immunology theory. The student has 60 minutes in total to answer. Questions cover the basic aspects of immunology cellular and molecular mechanisms as well as the main aspects of immunopathology. The questions will be as specific to evaluate the basic knowledge of the topic. This means that the student must know the key steps of immune response, the most relevant of the adaptive and innate immunity.

Student skills are assessed through various assessment tests during the course and a final oral exam (viva voce). The written exam consists of approximately 32 multiple-choice questions.
The oral exam covers all theoretical aspects of immunology. Students must be able to discuss the topic and connect and analyze the different parts of the subject.
Furthermore, students must have a thorough understanding of the most common immune responses, i.e., be able to link immune components and mechanisms to diseases. To test this ability, students will be asked to solve simple immunological or immunopathological problems, such as recognizing the immunological mechanisms underlying innate or adaptive immune responses, etc. The final grade is expressed on a 30/30 scale, with 18 being the lowest score and 30 being the highest.

IMMUNOLOGY
General aspects of the immune response: innate and acquired response (humoral and cell-mediated responses). Cellular and molecular bases of the immune response. Origin of the specificity and diversity of lymphocytes. Clonal selection.
Organization of the immune system: general characteristics of organs, tissues and cells. Ontogenesis, differentiation, phenotypic and functional characteristics of lymphocytic populations and subpopulations. Cellular interactions needed to generate an immune response. Communication systems between the cells (adhesion molecules, homing receptors, cytokines). Primary and secondary lymphoid organs. Distribution and re-circulation of immune cells. Organogenesis of lymph nodes.
Innate immunity: anatomic barriers, physiological and biochemical barriers, phagocytic and inflammatory barriers. The receptors of innate immunity. The molecular pattern associated with pathogens (PAMP) and molecular profile receptors (TLR, NLR, RLR, etc.). Phagocytosis: Cellular and molecular aspects of intracellular phagocytosis I and killing (oxygen and nitrogen-dependent and independent mechanisms) of neutrophils and macrophages. The lymphocytes of the innate immunity (ILC1, 2, 3). Natural Killer (NK) Cells: Origin and phenotypic characteristics. Recognition of target cells. Inhibitory receptors and stimulators. Isotypes, Allo-types, Idiotypes. Biosynthesis and catabolism. The structural and functional differences classes and subclasses. Monoclonal antibodies: concept, methodology, applications. Evolution of the immunoglobulin superfamily.
The antigen-antibody reaction: affinity, greed, specificity, kinetics of the antigen-antibody reaction. The T cell antigen receptor: molecular structure of TCR and CD3 gene products
The molecules of the major histocompatibility complex (MHC): Gene organization and polymorphism. Molecular structure and classification of gene products (class I and II MHC). Structure and function of the combinatorial groove. Role of Class I and II MHC molecules in antigen presentation. Non-conventional antigen presenting molecules.
The antigen presentation to the immune system. Detection of native antigen by B cells and MHC-restricted recognition by T cells. Main factors of independent cellular antigen differentiation. Positive selection and negative selection of T cells in the thymus. Selection of B cells in the bone marrow. Markers and methods for the study of lymphocytic development.
Mechanism of action of immunity receptors. Activation of T and B cells: definitions and general concepts. The signal transduction systems of the antigen receptors. Molecular mechanisms involved and the transcriptional factors in antigen-dependent proliferation and differentiation. Main accessory molecules, adhesion molecules and the main systems of co-stimulators and co-regulators participating in the process of lymphocytic activation.
Cytokines and their receptors: origin and molecular structure. The general characteristics of the cytokines. The expression of cytokines and the mechanism of action of cytokines in the transcriptional activation of target genes: the transduction pathways and the main transcriptional factors involved. Effector mechanisms of specific immunity
Effector mechanisms of humoral immunity. Cooperation between helper T cells and B cells: the cellular and molecular mechanisms for B cell activation as T dependent and T independent. Autoimmunity and Autoimmune diseases: more accredited etiopathogenetic hypotheses. General classification of autoimmune diseases. Relationships between HLA phenotype and frequency of autoimmune diseases. Experimental bases of Immunoregulation.
